About the Opportunity

This innovative clinic emphasizes personalized, proactive healthcare, enabling physicians to build meaningful relationships with a smaller patient panel (under 400 clients). The practice is supported by a high staff-to-physician ratio, ensuring comprehensive, team-based care.
Key Responsibilities
- Deliver comprehensive primary care with an emphasis on prevention, chronic disease management, and patient education.
- Collaborate within an interdisciplinary team, including nurse practitioners, registered nurses, dietitians, exercise physiologists, mental health counselors, and lab technicians.
- Utilize advanced technologies such as point-of-care ultrasound (POCUS), mental health tools, and genetic testing to enhance patient care.
- Engage in a practice model that prioritizes prediction, prevention, and early detection, leading to balanced and fulfilling clinical work.
Qualifications
- Certification in Family Medicine (CCFP) from the College of Family Physicians of Canada.
- Current registration with the College of Physicians and Surgeons of Ontario (CPSO).
- Minimum of one year of clinical primary care experience.
- Experience in collaborative, team-based primary care settings and chronic disease management is preferred.
