•      Georgia's non-expansion of Medicaid has left tens of thousands without coverage, making a sliding fee scale and FQHC mission critical to the region

•      Rural geography creates transportation barriers to care for patients in Baker, Calhoun, Dooly, Terrell, and other outlying counties

•      The region has been designated a Health Professional Shortage Area (HPSA), making it eligible for NHSC loan repayment for qualifying providers

Position Summary: Primary Care Physician (PCP)

They are actively recruiting a Primary Care Physician to join its Adult Medicine practice. The PCP will provide high-quality, comprehensive primary care to adult patients across Albany-area sites, with a focus on chronic disease management, preventive care, and acute illness treatment within the PCMH model.


This is an ideal position for a physician who finds deep satisfaction in longitudinal patient relationships, managing complex chronic conditions in an underserved population, and functioning as a trusted anchor for patients who may have limited access to care elsewhere.


Key Responsibilities

•      Provide comprehensive adult primary care including acute, chronic, and preventive services

•      Manage complex, multi-morbidity patients with conditions including diabetes, hypertension, heart disease, COPD, and mental health co-morbidities

•      Work within PCMH model, including care team-based coordination, care management, and population health initiatives

•      Collaborate with integrated behavioral health, pharmacy, and specialist teams embedded within the Clinic

•      Contribute to quality improvement, HEDIS measure performance, and UDS (Uniform Data System) reporting aligned with FQHC requirements

•      Participate in community outreach programs including mobile unit activities and health fairs

•      Supervise and support advanced practice providers (NPs/PAs) and medical residents as applicable

•      Serve rural outreach sites across nine-county service area as scheduling allows


Qualifications

•      MD or DO from an accredited institution

•      Board certified or board eligible in Internal Medicine, Family Medicine, or a related primary care specialty

•      Active, unrestricted Georgia medical license or eligibility to obtain

•      Demonstrated commitment to serving underserved or medically complex populations

•      Strong collaborative skills; experience in team-based care models preferred

•      Bilingual (English/Spanish) is a valued asset
